Official Capital One Classic Pro-Am Application

Ellendale Country Club, Wednesday, May 3rd, 2006 – Tee Times ranging from 7:30 – 9:30 am and 12:30 -2:30 pm.
Registration begins 1 hour before tee time; Lunch served 11:30 am – 1:30 pm.

Entry Fee: $1,000 per Team

Format: A shamble format will be played. The best drive of the group will be selected (Including pro). From this point all players will complete the hole with their own ball. 50% of handicaps will be used. One low net and one low gross will make up the team score. The pro’s score may only count once as either a net or gross ball.

Applications will not be accepted without payment. Entry deadline is, Friday, April 28, 2006.

Tournament Committee will review all handicaps and adjust if necessary.
 
I understand and agree that if, in the judgment of the Tight Lies Tournament Director, or his designee, adverse
weather conditions or other occurrences or conditions beyond the control of the sponsor of the Tight Lies Tour
renders commencement or continuance of the Pro-Am impractical then, in any of such events, the Pro-Am may be postponed, rescheduled or canceled, whichever be deemed appropriate by the Tight Lies Tournament Director, or
his designee; and, the sponsor will have no obligation to refund any portion of the entry fee remitted herewith by
reason of any such postponement, rescheduling or cancellation.
